Adolescent Opinion Survey

The Adolescent Opinion Study is a longitudinal study designed to examine predictors of risk behaviors among adolescents. The survey assesses attitudes, beliefs, coping strategies, and religious feelings. In addition, the survey asks about tobacco, alcohol, and marijuana use.

Only 9th grade students are eligible to participate in the Adolescent Opinion Study. Students will complete two surveys each year for three years. Each October students will receive a $35 gift card to Wal-Mart for completing a survey that takes no more than one hour to complete. Each April students will receive a $15 gift card to Wal-Mart for completing a survey that takes no more than twenty minutes to complete.  Thus, students who complete surveys at all six time points will earn $150 in Wal-Mart gift cards.

If you are a 9th grader and wish to participate, we MUST receive a consent form signed by a parent or guardian. We cannot allow anyone to participate without a consent form signed by a parent/guardian.
